The following process outlines the steps for scheduling an A1C Champions® program.

STEP 1: REQUEST A PROGRAM
At least 45 days prior to your preferred program date, complete and return
a Program Request Form by one of the following methods:
- Complete the Program Request Form.
- Call the A1C Champions® Business
Office at 855-A1CHAMP (855-212-4267) to have the A1C Champions® Program Request
Form emailed to you.

STEP 2: PLAN DETAILS
Approximately 45 days prior to your program, we will contact you to confirm
details, such as:
- Program location
- AV equipment needs (the healthcare
provider/program host or Sanofi US Sales Professional is responsible for providing
AV equipment)
- Anticipated number of attendees
(a minimum of 15 attendees is required to schedule a program)
STEP 3: RECEIVE AND USE MARKETING MATERIALS
Approximately 45 days prior to your program, we will send you a tri-fold
mailer, flier and calendar announcement for you to use to invite patients to the
program. To ensure that there will be a minimum of 15 attendees at your program,
we ask that you:
- Print, address, and send the tri-fold
mailer to your patients
- Print several copies of the flier
and post them
- Submit the calendar announcement
to your local media
STEP 4: RECEIVE PROGRAM MATERIALS
Approximately 2-3 weeks prior to your program, you will receive program materials,
if applicable.
STEP 5: CONTACT US ANYTIME
Approximately 1 week prior to your program, the A1C Champions® Business
Office will contact you to confirm that you have received your program materials
